[PW] Availability of Zurkowski report

Can anyone point me to an electronically-available version of Paul G. Zurkowski, The Information Service Environment: Relationships and Priorities (Washington, D.C.: National Commission on Libraries and Information Science, 1974)?  This was the report, it will be recalled, in which Zurkowski coined the term "information literacy".  I have searched the ERIC database, which has a record but indicates that it is not available from its resources, and I have searched the NCLIS web site and US National Archives, to no avail.  Google Books cannot assist.  For such a seminal report, it is most odd that it appears to be so inaccessible.
