Hi Nina,

According to Webster's New World Dictionary of the American Language, 
College Edition, the forms of address are: On the envelope: Full Name, 
Esq. (if a feller), American (or other) Consul, city and country or 
state; salutation (formal) Sir/Madam; (less formal) My Dear Sir/Madam; 
(in speaking) Mr. or Ms. (Surname).
