Would you include in this category the following?  W.S. Gilbert wrote
the librettos for all the Savoy operas.  But the cast was famous for
ad-libbing additional gags into the script.  Gilbert typically went
through these ad-libs when new editions of the librettos were prepared,
and either approved or rejected them.  Would you call those
ghost-written?

Even harder are cases like Nanki-Poo's response to Yum-Yum's declaration
of love for him in Act I.  Gilbert was also the director of the shows.
The actor playing Nanki-Poo had the line "Oh, rapture", which he
consistently over-emoted in rehearsal.  Gilbert finally said to him in
frustration, "Rapture, sir, yes, but modified rapture."  At which point
the actor delivered the line "Modified rapture!"  Gilbert was delighted
and left it in.  Did he write it?
