This is the ghost-writer credit (aka Lance Armstrong with Sally Jenkins).
It is NOT functionally the same as a dual author credit, so it shouldn't
be cited the same way.\

MLA citation style allows for flexibility for dealing with unprecedented
situations as long as the four basic information tags are there. Cite it
as Primary, Author, with Ghost Writer. Title. Place of Pub: Pub, date.

Fellow marsupials --
A student just asked me how to format a citation (MLA style) for a book
written by John Doe "with Jane Smith." I assume that this is treated as
any other two-author situation, but I haven't found evidence one way or
the other.
