Also, FWIW, my experience (in Washington State) has been dependent upon
the insurer and the pharmacy's particular policies. My wife and I were
forced to change pharmacies last spring. The former pharmacy accepted
one-year prescriptions, the new one accepts them with the proviso it can
contact the doctor "for renewal" during the third month.

My insurance coverage allows only monthly refills; my wife's (same
insurer, however different plan) allows up to three months of most
drugs. The policing of this seems up to the insurer's computer system.

FWIW I get my prescriptions in New Jersey. They are typically written 
for three months, refillable three times or, sometimes, for one month 
refillable 11 times.
