[PW] Need help with film ID

Hello, 
 
I realize these questions usually go out for books, but I'm hoping maybe
this crowd might recognize this vague description a patron gave me,
trying to re-locate a movie she borrowed from us previously.  Here are
the (sparse) elements I have to go on...
 
Movie probably released in the last 10 years.
 
Set in a US city.
 
Young (20's?) female protagonist has a terminal disease, maybe cancer or
a brain tumor, falls in love with young male character, in the end she
ends the relationship, needing to live out her time facing her illness
alone (or something like that, maybe she wants to spare him the pain of
loss).
 
The cover of the DVD she remembers having the female character in front
of a cityscape of some kind.
 
That's all I've got.  No names of actors, nothing more than that.  I've
power-searched IMDB like a madman with no luck.  I thought it might have
been "My Life Without Me", but there were some mismatches in plot that
didn't fit her recollection, and the cover image is wrong.  
 
Ring any bells out there?
