NASA also has a "Lessons Learned" database
(http://www.nasa.gov/offices/oce/llis/home/)
and has it since at least the 1990s.. "Lessons Learned" is a very popular
buzzword these days,
and it fits very well with the current interest in knowledge
management. The term may have
originated with the military: the DoD technical reports database at
http://stinet.dtic.mil/index.html
has several reports from the 1960s with "Lessons Learned" in the title.
