Well, the JSTOR article you sought was dated 1967, but a google-in-book
search indicates he used it prior to 1966 anyway...

The google snippet view for this one doesn't extend enough to tell me if
it's footnoted, but it should be worth checking.  Our copies of the
proceedings are in storage, but I could get them out and see if no
Wombat has a copy closer to hand.

started...?
