A few more fairly recent nutty dictators:

François Duvalier
Rafael Trujlllo
Yahya Jammeh
Francisco Macias Nguema
Mobotu Sese Seko
Kim Jong Il
Saddam Hussein

It's a lot harder to come up with elected heads of state/government who match Berlusconi, 
and that says a lot about you-know-who and the state of you-know-what.  American 
democracy has recently tried to push forward a few others of notable "minimal 
knowledgeability" like Christine O'Donnell and a certain Governor of Alaska.  Excellent 
sources for understanding the long-term reasons for this are Richard Hofstadter's 
"Anti-Intellectualism in American Life" (1962) and Susan Jacoby's "The Age of American 
Unreason" (2008.)

Rated very poorly by historians:  David Cameron (for self-inflicting the Brexit wound and not 
knowing what Magna Carta is), Tony Abbott, George W. Bush.  But none of these belong on 
the same list as mad monarchs and dictators.   (Well...  )
