Another approach is to go to Encyclopedia of Associations and check
theater associations.
The American Society for Theatre Research (ASTR) publishes the Directory
of Doctoral Programs in Theatre. Their Contact Us link is
http://www.astr.org/About/ContactUs/tabid/114/Default.aspx. They should
be able to answer the question.

The earliest US PhD I can find in Digital Dissertation with dept. name
or subject name given as either theater or theatre is this one, but I'm
not sure the "subject" appears in the original dissertation as opposed
to being DD's after-the-fact addition:
