The earliest US PhD I can find in Digital Dissertation with dept. name
or subject name given as either theater or theatre is this one, but I'm
not sure the "subject" appears in the original dissertation as opposed
to being DD's after-the-fact addition:

Yale and the University of Chicago each show several dissertations with
theater as a given subject in the first decade of the 20th century,
with same provisio.  By around 1910 they start to become rather plentiful.
