Shari:  Altho' you asked for "once mortal from assuming,"  I'm guessing that
you meant "mortal FORM assuming."  If so, here is an excerpt from an article
in Christian Century, June 4, 1997, entitled "Beyond the worship wars."  It
discusses new words for old hymns.  I don't know if this might be a clue to
what you're looking for, but it does include some of the word combinations
you mentioned:

One Christmas morning I heard a choir sing a new version of "Still, Still,
Still." The original Austrian carol urges us to be still while the
Christchild sleeps, to hear angels singing, to receive God's gift of
forgiveness. One verse in an older translation is "Full, full, full,
Immortal love so full / That God his heav'nly throne forsaking / mortal form
for us is taking. / Full, full, full, Immortal love, so full."
