Sheri,  It's not in the 1982 Hymnal of the Episcopal Church.  This 
includes an index of the first lines of hymns.  Is there a 
possibility that your line --  "Immortal love once mortal form 
assuming"? -- could be the first line of a second or subseqent verse?
