[PW] revised Hugo Awards ballot, after two ballot-item removals at the creators' request

As this is the third and perhaps last year that the bloc voting on the part of the "Sad Puppies" and "Rabid Puppies" slates is in evidence, and two creators, John O'Neil of BLACK GATE and Thomas Mays, were nominated with the aid of bloc voting without their consent nor approval, and both have asked that their work be removed from the ballot, and it has.
